When nfc proto id is using, nfc_proto_register() return -EBUSY error
code, but forgot to unregister proto. Fix it by adding proto_unregister()
in the error handling case.

In preparation for changing how struct net is refcounted
on kernel sockets pass the knowledge that we are creating
a kernel socket from sock_create_kern through to sk_alloc.

Several files refer to an old address for the Free Software Foundation
in the file header comment. Resolve by replacing the address with
the URL <http://www.gnu.org/licenses/> so that we do not have to keep
updating the header comments anytime the address changes.

With calls to modular infrastructure, these files really
needs the full module.h header. Call it out so some of the
cleanups of implicit and unrequired includes elsewhere can be
cleaned up.
